This is our time to really learn you and your goals! Our specialized nurse will reach out to discuss any symptoms of low testosterone you are currently experiencing and what areas you would like to focus on optimizing. From there, we will clarify our process, and discuss next steps!
BLOOD WORK
Our number one goal at Form MD Performance is to optimize your health while remaining medically safe! In order to do this, we require an initial set of blood work. Blood work allows us to objectively learn your health while exposing areas you can optimize. This can be scheduled with our office at YOUR convenience!
PROVIDER CONSULTATION
You will meet with Dr. Pollei, one-on-one, to discuss your goals, existing symptoms, review your labs, and work together on creating a customized package just right for you!
SIGN UP & SUBSCRIBE
From here, Form MD Performance will completely take over! With our subscription model, you will be shipped your injections each month.
MAINTENANCE & OPTIMIZATION
If there are any adjustments to your existing membership, our team is here for you! We are available to you any time. We require blood draws to ensure safety and track your progress, as well scheduled appointments with Dr. Pollei bi-yearly for a personal base touch – all at your convenience!
