Clinical Director

Lany is a highly experienced nurse with a wide breadth of experience ranging from acute bedside care at Cedars-Sinai Medical Center, to Plastic Surgery and Aesthetics in Beverly Hills, to outpatient concierge medicine and IV therapy here in OC. She blends the incredibly valuable skills of communication & patient care planning with medical operations.

Lany is the clinical touch point in your personalized patient care. She is passionate about all things health and wellness and creating relationships with her parents.

Operations Manager

Austin comes from an impressive background in health and wellness. With extensive experience in setting up and operating businesses during their initial stages, Austin has a proven track record of optimizing operations and driving organizational success. Having held key positions at Xponential Fitness, where he led the inaugural openings of a brand-new group strength training concept in Southern California.

Austin is excited to enter into another facet of the health and wellness industry and help optimize patient’s lives with the services provided by Form MD Performance. His goal is to provide excellent communication and support service to every patient.
